For your first visit


  • Physical therapists in NC are not required to have a physician's referral to evaluate and treat patients.  However, if you have been referred by a physician, please bring the referral and/or contact information with you.
  • Please download and print the intake forms below or arrive 15-20 minutes early to complete paperwork at the office.
  • Please bring or wear loose comfortable clothing. Shorts and tank tops may allow me to evaluate and treat your condition more effectively. (Gowns will also be available.)


  • If possible, do not wear oils or lotions on your skin for improved results with Myofascial Release treatments.
  • If you are seeking treatment for Women's Health issues including pelvic pain, coccydynia, and incontinence, an internal assessment of the pelvic floor muscles may be performed in order to provide the most effective treatment of these issues. Please feel free to call if you have  questions regarding the pelvic floor physical therapy assessment.
